Dear Parent or Guardian:
The Irvine Unified School District will once again require residency verification for all IUSD students enrolling for the 2012-13 school year. This process is an annual legal requirement in California, and it helps our district ensure that all available resources are spent efficiently and effectively to the benefit of Irvine families. Moreover, these additional steps will help provide a full and precise accounting of our student population as we review staffing for the coming year.
By completing the following two forms, you will be making a formal declaration that your child lives within IUSD's attendance boundaries.
1. Residency Verification Form (with fillable fields)
2. Residency Affidavit Form (with fillable fields)
For your added convenience, they are also linked above, allowing you to fill them out and print them at home. Completed residency forms should be turned in to Venado Middle School by April 1, 2012, along with proof of the parent's identification and two documents that verify residency. A list of acceptable documents is included on the Residency Verification Form.
Note: The deadline for turning in Residency Verifications has been extended to Monday, April 23. Any verifications not received by this date will be referred to Student Services. |
